Qualifications

I currently conduct independent living skills training in the consumers home and community settings. I teach advocacy skills when the client is making doctor appointments. I like to make learning based on every moment of interaction with the client as based on their learning plan according to the client's goal and objectives set and agreed upon by the client’s (ISP). I have knowledge of leading, organizing, and manage sessions based on the individual personal IPP and assessment needs. I implement training tools and techniques to facilitate training effectiveness per client or group. I work well with others and or independently. I provide support to obtain and maintain community services needed for the consumer. I prepare thorough documentation and maintain accurate and systematic records. I communicate clearly with other health teams, supervisors and service coordinators. I am able to establish and maintain effective work relationships with my peers and special populations.
