In my capacity as a Clinical Assistant, I operate within the full scope of a physician extender under the supervision of the internal and family medicine physician team within the acute medicine department. As delegated by attending physicians, my responsibilities encompass:

• Conducting comprehensive and collaborative assessments, making diagnoses, and formulating treatment plans for admitted patients.
• Ordering and interpreting necessary tests for accurate diagnoses and treatments.
• Collaborate with the medical team to implement management plans.
• Monitoring daily progress and managing new complications as cases evolve.
• Participating in daily discharge planning rounds to collaboratively review patient cases with the medical team.
• Formulating suitable discharge plans for patients to ensure seamless continuity of care and patient safety.
• Prescribing pharmaceuticals and performing procedures within the legislative scope of practice.
• Gaining exposure to a diverse array of pathologies, including but not limited to COPD, Pneumonia, CHF, hyponatremia, fall management and prevention in the elderly, delirium, hepatic encephalopathy and more.

• Providing collaborative clinical care to patients as a part of a clinical team that comprises 4 family physicians, 5 nurse practitioners and 4 clinical assistants
• Diabetic foot exams and foot care teaching
• Counseling patients about blood tests and imaging results under physicians' guidance
• Promotion of periodic health maintenance and preventive screening tests
• Phlebotomy, ECG, Ear Syringing, immunizations and injections of medications.

During this period, I was working as a hospitalist.
Hospitalist Duties:

• Conduct consultations and perform thorough medical evaluations for new admissions.
• Collaborate with the medical team to establish and implement management plans.
• Conduct daily rounds, documenting progress and adjusting care plans as needed.
• Facilitate discharge planning to ensure continuity of care post-hospitalization.
• Provide emergency care and follow-up on ICU cases when required.
• Offer pre-operative and post-operative care, including assistance in normal and caesarean deliveries.
• Perform neonatal resuscitation as part of the comprehensive care approach.

• Counsel patients on test results under physician guidance.
• Promote health maintenance and preventive screening tests.
• Conduct pre-consultation assessments and evaluations.
• Collaborate for referrals and prescription renewals with healthcare providers, hospitals, and imaging centers.
• Perform phlebotomy, ECG, ear syringing, spirometry, and COVID PCR testing.
• Administer immunizations, parenteral injections, and intravenous infusions.
• Update patient charts and document medical history in the Electronic Medical Records (EMR).
• Manage patient appointments and handle a high volume of emails, phone calls, and faxes.

• Assisting the psychiatrist and parents/patients in completing variable psychiatry assessment forms for ADHD (SNAP-IV), depression, anxiety for both children and adolescents
• Managing phone calls, appointments, emails and faxes and prescriptions.

In my professional role, I fulfilled responsibilities as a hospitalist and a community family physician for an outpatient clinic associated with the hospital. Additionally, I dedicated time each month to volunteer and offer healthcare services in rural communities.

Hospitalist Duties:

Community Family Physician Duties:

• Manage a roster of 100 families, equivalent to approximately 500 patients.
• Assess, evaluate, and manage a diverse spectrum of medical conditions in the outpatient setting.
• Request and interpret investigations, facilitating timely and accurate diagnoses.
• Promote health through patient education and encourage primary prevention methodologies.
• Provide prenatal and postnatal care, as well as conduct well-child examinations.
• Facilitate referrals to specialists when specialized care is warranted.
• Report births, deaths, and communicable diseases to contribute to public health initiatives.

• Patient centered approach to history taking, physical examination, evaluation of admitted patients, and corresponding documentation of their daily progress in the electronic records
• Discussion of cases and setting up a management plan
• Reviewing and interpretation of test results and imaging reports
• Providing comprehensive and collaborative care in admitting, assessing and stabilizing emergency cases
• Assisting in patient education and advocacy of health promotion and prevention of medical conditions
• Management of acute cases.
• Evaluation and follow up of patients at a long term care attached to hospital.
• Volunteered to serve at Port Hope Simpson as part of the medical team, providing care to a small indigenous community

My initial practice was mainly a hospitalist, and sometimes I provide walk-in consultations at outpatient clinic attached to the hospital.

Hospitalist Duties:

• Conduct daily rounds, documenting progress and adjusting care plans as needed.
• Facilitate discharge planning to ensure continuity of care post-hospitalization.

• Offer pre-operative and post-operative care, including assistance in normal and caesarean deliveries.
• Perform neonatal resuscitation as part of the comprehensive care approach.

• Assisting with patient care activities under supervision, such as obtaining medical histories, physical examination.
• Collaborating with the healthcare team on patient management plans.
• Contributing to patient education materials or presentations.
• Engaging in educational activities, including lectures, workshops, and grand rounds.
• Observing and potentially assisting with medical procedures and surgeries ( colonoscopy for uncomplicated sigmoid volvulus, paracentesis, and ultrasound guided central venous line insertion. )

• A 12-month rotation, with firsthand experience, in various specialties including Emergency Medicine, Anesthesia, Internal Medicine/Family Medicine, Obstetrics/Gynecology, Oncology, Neurology/Psychiatry, General Surgery, Pediatrics, and I received optional extra training in Neonatology.
