● Assess individuals with developmental disabilities exhibiting complex behaviours. Ages range from 8 to 50+ year old individuals in residential care and day program settings. Examples of developmental disabilities supported include Autism Spectrum Disorder (ASD), Fetal Alcohol Spectrum Disorder (FASD), Down's Syndrome, and genetic disorders.

● Manage a caseload that would range from 30-35 clients
● Liaise and collaborate with various stakeholders, such as community support workers, families/guardians, caseworkers, doctors, and teachers
● Write documents such as wellness support plans, planned positive and restrictive procedures, risk assessments, protocols, and functional assessments.

● Attend meetings for clients where advocacy for their supports and wellbeing is necessary, such as psychiatrist appointments, neurologist appointments, house meetings, and school meetings
● Create and review tracking sheets for an individual's specific behaviours, as well as for certain staff responsibilities
● Facilitate meetings for assessments, wellness support plans, as well as training such as new client orientations
● Review and analyze critical incident reports with follow up feedback or recommendations if necessary, to ensure safety for the client, staff, roommates, and the community
● Create individualized strategies and approaches and help staff with ways to implement these strategies to further support their clients. Examples of strategies may include using visual communication, schedules, integrating sensory needs, ideas for meaningful engagement, and social stories
● Conduct observations for clients in several settings such as school, day programs, during an outing, or in their home

● Offer recommendations for environmental adaptations and create orders/purchases required for any adaptations if necessary

● Assist with organizing company events such as swim meets or art shows, with making recommendations relevant to clients' needs when attending such events

● Keep abreast of any new developments in the field of developmental disabilities and mental health, and continue to be well-informed of best practices, recent research, and training from external resources and organizations

Mentor (Community Disability Practitioner)

● Worked with a group of four individuals with developmental disabilities to facilitate community access, employment, and support with individual goals under the Framework for Life program
● Supervised two other Mentor Assistants (community support workers) to ensure proper implementation of programming, advising of strategies, and review of their weekly data and observations through one-on-one meetings
● Reviewed and approved Mentor Assistants’ hours through Payworks

● Logged notes for clients to share with other support workers, such as daily activities, foods eaten, or notable events

● Administered oral medications
● Documented daily activities, daily lesson plans, weekly and monthly schedules, goals worked on or achieved, and tracking of behaviours through Microsoft Excel and uploaded to a virtual platform
● Liaised with various stakeholders such as several community facilities, volunteer and employment centers, residential Assistant Managers (team leads Entrust homes), Service Managers, and guardians
● Transported clients in my vehicle and participated in activities, appointments, and community outings with them
