Job description:
- Assess resident’s physical, emotional, social, mental and medical status.
- Keeps up-to-date reporting all throughout the shift.
- Ensure resident’s safety by checking resident’s status

- Create ez-site to resident’s who will start on clysis and those who will receive medications subcutaneously for a long period of time
- Coordinate with other medical and non-medical staffs pertaining to resident’s welfare and progress
- Assist in feeding and taking care of residents
- Attends and participates to programs, seminars and/or teaching to further career growth and development
- Suggest and/or recommend ideas and vision for facility improvement/s and/or advancement
- Administer medications per doctor’s order. Know the indications, correct dosage, side effect and adverse effects of medications.
- Accomplish forms in admitting residents and collects pertinent data of residents from relative/s or significant other/s
- Assess and change dressing/s as per facility protocol
- Do vital signs and collects specimens from residents as the need arises.
- Informs charge nurse or care manager regarding equipments that are not functioning or not of use to the resident or staffs in the unit.
- Take note of and reports incidents to the charge nurse or care manager , also documents it.

Course in the ward

- Administer medications per doctor’s order. Know the indications, correct dosage, side effect and adverse effects of medications.
- Do vital signs and collects specimens from residents as per doctor/NP’s order.
- Inform charge nurse or care manager regarding equipments that are not functioning or not of use to the residents or staffs in the unit.
- Take note of and report incidents to the charge nurse or care manager then document.
- Do pertinent responsibilities regarding post mortem care of residents. Coordinate with funeral home regarding funeral arrangements/death certificate. Inform family members of resident’s passing.
- Assist with transfer of residents within or outside of facility as per facility policies and guidelines.
- Coordinate with other facilities regarding resident’s procedures to be done, follow-up results and inform NP/MD.
- Orient new staffs and/or students as assigned by the charge nurse or care manager.
Attend family conferences as part of the nursing team. Update resident’s family member regarding current status. Give suggestions and solutions as per family’s concern.
- Process/carry out orders made by NP/MD FOR POSSIBLE/ACTUAL ADMISSIONS
Review/match potential admissions based on resident’s profile. Coordinate with Transition coordinators, faciliy and family members to facilitate admission. Coordinate with RCMs, RNs, pharmacist, doctors/NP and sending facility/hospital. Do pre-admission interview to family members - tour them around the facility, show them where each departments are located and introduce them to staff members. Accomplish forms in admitting residents and collects pertinent data of residents from family member/s or significant other/s.
WOUND CARE
Attended wound seminar. Participated in annual wound prevalence rounds by Hillrom at facility. IAssigned as a wound champion in our unit. Assess and change dressing/s as per facility protocol.
FALLS
-Do a thorough assessment on resident’s injuries/fractures on fall incidents - accomplish SPLATT form. Monitor resident’s neurovitals, pain, ROM and mobilty for 48 hours. Facilitate transfer/call 911 for emergencies. Inform family members, AHN and MD about fall incident.
COVID RELATED CONDITIONS
- Monitor and report resident with possible Covid symptoms to AHN/RCM/ICP - do resident naropharyngeal swab as needed.
- Set-up isolation/PPE materials and be diligent in observing isolated residents.
- Act as PPE coach to colleagues and family members as needed.
- Screen visitors by the facility reception and/or staff entrances.
- Monitors any side effects post Covid/Influenza immunizations.
- Serve as a role model and mentor for health sciences students.
