Acute Rehab/Med-Surg-Tele:
—Assisted patients with maintaining personal hygiene, and ensuring safety and comfort.
—Administered medications through various routes, including PO, subcutaneous, intramuscular, and g-tube/NG tube.
—Accucheck. Pain management. Ostomy/wound care.
—Performed routine assessments, documented vital signs and patient condition changes, and communicated clinical updates to RN, physician, and family. Followed up with pharmacy of new medication orders.
—Monitored laboratory results, and ensured timely communication of lab abnormalities to physicians, and document follow-up actions.
—Collected and send specimens to the lab.
—Assisted with medical procedures including straight catherization, bladder scanning, rectal tube, NG tube insertion, and blood transfusion.
—Managed IV fluid therapy and replaced primary lines, and changed peripheral and midline dressing.
Acute Rehab:
—Assisted with writing weekly patient summaries including overall outcome goal, interventions, progress, and recommendations. —Offered emotional support and motivation to patients during recovery to help them regain independence.
—Educated patients and families on disease management, pain management, medication use, and fall and injury prevention.
Med-Surg/Tele
—Supported respiratory therapists by performing suctioning, oral care, and monitoring ventilated patients.
—Responded to ventilator/BiPap alarms and participated in emergency situations.
—Monitored patients with cardiac conditions, especially with those on telemetry, and document and report abnormalities on their heart rhythm to RN and physician.
Provided compassionate patient care, assisting with activities of daily living including bathing, grooming, toileting, eating, and mobility. Vital signs. Patient support and advocacy.
