GENERAL ACCOUNTABILITY: Under the direct supervision of the Clinical Supervisor, the Addiction Crisis Worker is responsible for the physical well-being of the clients and the environment of the non-medical detoxification unit. Key responsibilities include admitting and assessing individuals suffering from substance abuse, safely managing withdrawal, and providing support, service planning, and stabilization; performing all routine duties in maintaining a residential program.

SPECIFIC ACCOUNTABILITIES:

• Screens prospective clients, admitting those who meet admission criteria, and referring others to the most appropriate services.
• Assesses the need for medical attention or psychiatric intervention, and completes the referral.
• Establishes and documents clients' general condition upon admission. Closely monitor clients' condition and behavior during the observation period.
• Safely manages aggressive clients and/or clients in crisis. Applies training in NCI and CPR as needed.
• Maintains a daily record of clients’ progress by documenting in individual files and the daily logbook.
• Interviews clients, utilizes the client service plan to establish client needs, and develops a client-centered action plan.
• Provides ongoing, nonjudgmental support and encouragement. Treat all clients with respect and dignity.
• Provides education about addiction and recovery, utilizing videos, one-on-one, or group discussion.
• Provides access to self-help groups and makes referrals to appropriate external services.
• Prepares clients for discharge. Performs relevant functions of file closure and return of possessions.
• Ensures that the hygienic level of cleanliness is maintained at the center outside the normal regular working hours of the cleaner.
• Is prepared to respond to emergency situations (i.e., fire, disaster).
• Actively participates in staff meetings and training sessions.
• Performs all aspects of the job in a manner that protects the confidentiality of clients, staff, and the department.
• Maintains ethical standards and professionalism in all interactions with clients, co-workers, and agencies.
• Performs other related duties as assigned.

• Responsible for following all Occupational Health and Safety regulations, as well as relevant organizational policies and procedures, to ensure personal safety and the safety of others.
• Participates in monthly safety inspections, emergency drills, and training programs as required.
• Reports any equipment, safety concerns, or hazards to the supervisor.

Client safety:

• Supports and contributes to a culture of safety, and promotes client safety.
• Responsible for following organizational policies and procedures.
• Identifies and informs managers and supervisors of client safety problems and concerns, including actual and potential adverse events and incidents.
• Assesses client safety issues and concerns, and actively participates in improvement activities.
• Utilizes effective and appropriate mechanisms for the transfer of information at interface points.
• Utilizes verification processes and other checking systems for high-risk care/service activities.
• Participates as required in training on targeted client safety focus areas.

• Weights taking, bath, who needs assistance with meal prep or for feeding, who is on toileting routine that requires support.)
• Assist or provide personal care or activities of daily living for clients promoting independence and wellbeing under the direction of an RN or RPN
• Documents and reports daily client activities, comfort, safety factors, emotional/behavioral changes, change in skin appearance, and results of enema/suppository, care, and treatments
• Review day planners for appointments and who needs to be ready for specific time
• Make bed and change linens as needed
• Assist clients in maintaining personal hygiene through bathing, grooming, and dressing tasks
• Maintained accurate records of patient care, progress, and concerns, contributing to effective communication among healthcare team members
• Assist clients with transfers and ambulation including the use of walkers, transfer belt, cane and wheelchair
• Help prepare meal trays for clients such as opening lids, cutting meats, and others
• Accompany the medically stable client to internal appointments
• Promotes a high quality of life for the clients through the awareness and application of Senior Friendly, Dementia Friendly Care

• Receive and admit clients according to designated procedures.
• Maintain the client's confidentiality.
• Assess and assist clients' personal needs, such as personal hygiene, nutrition, medical care, and emotional support.
• Support the harm reduction model of the client.
• Communicate effectively, including active listening and problem-solving.
• Monitor client activities, and provide effective intervention in crisis situations.
• Monitor incoming and outgoing individuals, and visitors appropriately.
• Work effectively and cooperatively with other staff, volunteers, and clients.
• Update and maintain accurate client information using the HIFIS database.
• Assist in cleaning, laundry, and other housekeeping activities, as required.
• Prepare, cook, and distribute meals with safe food handling and preparation, safety and health standards, and with assistance from volunteers and clients.
• Ensures the security and safety of the building, staff, and clients.
• Provided crisis intervention support when needed, deescalating situations while prioritizing the safety of all involved parties.

• Support and build caring relationships with individuals with developmental and intellectual disabilities, such as Autism Spectrum Disorder, Down syndrome, Parkinson's disease, and developmental delays.
• Promotes the independence of people, supported by encouraging and supporting individuals to strive to reach their full potential.
• Provides the person supported the opportunity for dignity of risk and informed choice-making in all aspects of living, and the opportunity to exercise control over his or her environment, allowing natural consequences and intervening only for reasons of safety of self or others.
• Facilitates and supports the development of meaningful relationships for the individual, maintaining involvement and communication on an ongoing basis.
• Maintain the confidentiality of records regarding the treatment of the person supported.
• Administers medications safely, accurately, and in accordance with the policy, and ensures accurate documentation with the supervision of senior staff or my mentor.
• Assist individuals supported with proper grooming, bathing, feeding, and transfers using a mechanical lift.
• Provides and ensures the safety of individuals in the group home.
