Bachelor of Science in Nursing RN, currently seeking to obtain Virtual Care Coordinator position with the Government of Prince Edward Island.

· Within this role, I independently worked throughout the community providing in depth comprehensive assessments to clients in their homes
· I was responsible for implementing care, creating care plans, and following treatment plans
· Strong assessment and nursing intervention skills are required in order to provide physical, mental, and social assessments with patients, and their families
· This position allows to complete nursing procedures, dressing changes, IV therapy, and injections

· Mini mental status exams (MMSE) and completing SAST safety assessments while in the home to deem whether a patient is safe in their living environment
· Having the ability to assess and recognize deficits in normal functioning and be able to encourage self care and independence
· Being able to provide care to Palliative clients, discuss advanced care planning, and documentation of GOC forms with clients and their families
· Completing the ESAS screening tool to evaluate palliative clients

· Supervising home support workers on weekends
· Educating clients and families on health promotion and illness prevention
· Advocating for clients, and being a resource for community services that are readily available
· Work collaboratively with multiple disciplines- Care Coordinators, Social Workers, Dieticians, Nurse
· Practitioners, and Physicians to ensure clients remain in their home for as long as possible.
· Updated and developed care plans to establish goals based on patient's nursing diagnosis.
