· Communicate effectively to all team members to ensure continuity of care and updating care plans based on client needs
· Teach, supervise and counsel the client and family members and other members of the health care team, regarding the nursing care needs and other related problems of the home care client
· Observe and report to the physician the client's response to treatment and any changes in the clients condition as needed
