Prepare and present monthly webinars and onsite education for CCIS users

• Monitor and respond to CCIS training questions
• Problem solve, track and escalate complex issues affecting data quality
• Prepare and update CCIS educational documents
• Provide recommendations and feedback to product manager for system enhancements
• Liaise with internal and external stakeholders regarding policy development and program planning
• Act as subject matter expert for both internal and external stakeholders locally and provincially
• Provide training to external stakeholders regarding Personal Health Information Protection Act (PHIPA).

• Provided group education and individual counseling for patients with type 1 and type 2 diabetes from diagnosis through disease process using empowerment strategies, motivational interviewing etc.
• Provided diabetes education for health care professionals including RN’s, physicians and pharmacists.
• Developed collaborative partnerships within community with local Family Health Teams, physicians, chiropodists, pharmacists and hospital staff
• Coordinated scheduling, educational events, new staff member orientation and student nurse placements
• Conducted bi-annual performance appraisals and provided daily counseling, guidance and support to team members
• Oversaw and managed a number of clinical and clerical staff while maintaining a high level of staff satisfaction and professionalism
• Developed new hire RN/RPN diabetes orientation for Joseph Brant Hospital

• Reduced hospital readmission rates through effective discharge planning and follow-up care coordination.
