Schedules and assists patients/members with program enrollment process, including patient/member education on available programs, eligibility and related requirements.
Coordinates with ODCHC staff, community partners, and others as appropriate to identify patients/members for program enrollment.
Refers patients/members to other inter-agency departments or community health/ social services as needed.
Attends meetings/trainings and reviews literature to maintain a current knowledge of programs and services available and pertinent regulations and eligibility requirements.
Coordinates with agency staff for publicity and outreach initiatives for availability of programs to patients and the community.
Proposes program initiatives for increased program enrollment.
Coordinates with other community agencies as appropriate for increased enrollment program capacity.
Tracks enrollment and program data.
