Maintaining statistical data, monitoring program activity levels, efficiency, budget, trends, and impacts.
Coordination of client needs assessments using INTER-RAI CHA, liaison with Home and Community Care Support Services (HCCSS) to support quarterly care planning and meet performance targets.
Develop, coordinate and facilitate wellness programming and workshops, training seminars, activities and events for both caregivers and client support groups.
Collaborate with the volunteer coordinator and HR departments to source volunteers for program needs and client care.
Maintain good relationship and liaison with community health care providers through monthly meetings and check ins relating to client care and care planning. Provide clients and caregivers with timely and accurate referrals to additional community services as needed.
