• Follows all Axis Family Resources Ltd.’s policies and procedures (Agency Manual) and the
Specialized Supportive Housing Program procedures and protocols.
• Follows and upholds Agency values of respectful communication, positive work environment,
quality/excellence of service and effective operations.
• Recognizes, analyzes, and deals with potential emergency situations such as person served
aggressive behaviour to ensure no harm comes to the person served and/or public. Reports
problems to supervisor.
• Participates in the assessment, goal setting and program planning (Integrated Case
Management) for individuals living in a specialized supportive housing. Documents,
implements, and provides input into the evaluation of the program.
• Provides life skills training such as meal preparation, housekeeping, personal care skills and
implements personal service plans.
• Participates in various person served focused activities in accordance with care plans.
• Administers medication to person served in accordance with established policy.
• Assists with case management by identifying potential problems and reporting any difficulties.
Provides input to the team about the development of appropriate program plans to achieve
person served objectives. Contributes to the evaluation of person served progress and prepares
reports.
• Provides emotional support and feedback to person served.
• Transports and assists person served to appointments, shopping, or leisure activities.
• Performs resource maintenance and housekeeping duties such as laundry, sweeping, mopping
floors, mowing lawns, inventory, shoveling snow, shopping cleaning equipment and food
services.
• Documents as required on Sharevision, and on person served file on every shift.
• Identifies and links social, economic, recreational, and educational services in the community
that will meet person served needs. Maintains liaison with other agencies, professionals,
government officials and the community.
• In some specialized supportive housing programs, assists person served with activities of daily
living such as feeding, lifts, transfers, hygiene, grooming and toileting. This may include transfer
of function duties such as gastronomy feeding, tracheostomy management, suction and bowel
management.
• Other duties as required.
• participates in assessment, goal setting and personal planning such as Person-Centered
Planning. Documents and implements the plan. Provides input into the evaluation of the
plan,
• assists individual(s) to function more independently in their own homes and in the
community. Assists individuals with daily life skills, social skills and / or behavior
management. Teaches and assists individuals with activities such as grooming, basic
cooking, money management, shopping, household safety, pet care. Facilitates physical,
recreational, educational, social and vocational activities,
• recognizes, analyzes and deals with potential emergency situations such as negative or
aggressive behaviors to minimize potential harm or negative perception to the individual
and / or public. Reports problems to the Program Coordinator,
• administers medication to individuals in accordance with established policy,
• ensures health and safety standards are maintained in accordance with established
policies,
• provides individual(s) with needed emotional support and to actively encourage the
development of friendship and other relationships that will augment staff and family
involvement,
• encourages and assists individual(s) to make use of the recreation, social, economic
educational and public transportation community resources that will best meet needs and
preferences,
• provides a positive role model for the individual(s),
• ensures required documentation is factual, accurate and professional,
• participates in training new and casual employees, volunteers and practicum and / or
summer students by demonstrating work and equipment procedures,
• participates in program and committee meetings. Maintains current knowledge of the
Society’s policies and procedures and
• performs other duties as required.
• Receives laboratory samples, laboratory requisition, extracts and labels laboratory specimens.
• Performs routine and special laboratory examinations.
• Responsible for the blood screening of blood donors, blood component preparation cross-matching, and transfusion reaction workouts.
• Keeps blood samples that need further workup or investigation.
• Analyze the suitability of the specimen prior to processing.
• Perform physical, chemical microscopic and microbiological examinations on blood and other body fluids.
• Carry out proper storage and issuance of blood and blood components.
• Observe all protocols adopted when running a test and quality control in all test runs.
• Writes test results on official result forms and records test results.
• Validates all test results.
• Performs other related tasks.
