• Work as part of a multidisciplinary team to provide pharmaceutical care to patients in an outpatient setting; provide therapeutic recommendations to the team with respect to medication therapies

> Determining if medication is suitable for HPTP by reviewing CSP database, cost, and supply issues

> Analyzing stability, sterility, and safety data to create manufacturing worksheets for IV preparations

> Performing calculations required to program IV pump

> Enable discharge by meeting directly with patients/families to provide program and medication education, ordering required supplies, setting up funding, and communicating plan to outpatient pharmacy

> Monitoring ongoing therapy for effectiveness and adverse effects

• Provided direct patient care to the outpatient population of Alberta Health Services, providing specialized care unavailable in the community and ensuring continuity of care at discharge
• Directly involved in the dispensing of medication and supplies for the HPT program. Responsibilities included interpretation of IV prescriptions and CSP worksheets, calculations required for pump program and TKVO, signing off on IV staging and final product integrity, and collaboration with ID pharmacist to facilitate discharge
• Practiced under full scope by means of initial access prescribing, extending of prescriptions, administering injections, and completing care plans
• Facilitated specialty programs and coverage such as the SHCDP, SAP, biologics, and compassionate supply
• Daily compounding for pediatrics. Involved in all steps of compound procedures including formula calculations, interpreting stability data, physical preparation, proper labeling and storage, and checking final product integrity
• Worked collaboratively with physicians, clinical pharmacists, social workers, dieticians, and support programs
