  • Work as part of a multidisciplinary team to provide pharmaceutical care to patients in an outpatient setting; provide therapeutic recommendations to the team with respect to medication therapies
  • Provide education with respect to drug therapy and disease state management through direct patient engagement

  • Prescribe, monitor, and adjust medication therapies including valganciclovir, vancomycin, aminoglycosides, and acyclovir

> Determining if medication is suitable for HPTP by reviewing CSP database, cost, and supply issues

> Analyzing stability, sterility, and safety data to create manufacturing worksheets for IV preparations

> Performing calculations required to program IV pump

> Enable discharge by meeting directly with patients/families to provide program and medication education, ordering required supplies, setting up funding, and communicating plan to outpatient pharmacy

> Monitoring ongoing therapy for effectiveness and adverse effects
