Akiko Kobayashi

• Present on unit and work with frontline staff, provide direct supervision and support for high-quality performance and evidence based best practice in resident care.
• Ensure current practices align with divisional/home policies and procedures.
• Apply strong knowledge about nursing documentation that meets professional standards and requirement by City of Toronto divisional policies, provide education and coaching to frontline staff
• Utilize data and various reports (PCC reports, Inspection Protocols, and other audits) to monitor nursing care on units, provide education to staff for quality care and accurate documentation.
• Enforce Infection Control and Prevention Practice on unit and provide staff with most updated IPAC measures in timely manner
• Identify gaps and weakness in staff/team performance, provide education and training to ensure residents' safety. Conduct investigation as appropriate.
• Review and evaluates procedures/practices, identify gaps, work with multidisciplinary team and utilize resources for improvement as required
• Lead Falls Prevention and Skin & Wound programs, provide support to clients and frontline staff. Also a member of Palliative Care Committee.
• Compile statistics in home for Clinical Programs, analyze data and provide information to multidisciplinary team and various committees
• Oversees unit operations, resources are utilized efficiently and appropriately to maximize quality of residents' care.
• Address residents'/family concerns and complaints, follow up, and implement interventions/remedies as necessary
• Promote teamwork and respect to others, foster safe and inclusive working environment
• Understanding of High Intensity Needs Funding. Provide information to team and conduct follow up on applications as required

Falls, and Skin and Wound Program Lead

• Provide assistance and support to nursing staff for Falls Prevention, Skin and Wound - assessment, interventions, evaluation, and multidisciplinary team collaboration
• Provide education and training to registered staff and PSWs as assigned

Other Roles

• Assist residents, families, and staff during admission process, complete intake and admission assessments, complete RAI-MDS assessment

Registered Nurse-In-Charge

• Report to on-call managers and act as managers’ delegate after hours and weekend
• Familiarize self with updated policies and procedures to guide staff nurses, including areas of IPAC, medication administration, and risk management
• Report client’s concerns &complaints to respective manager(s) and initiate actions as appropriate
• Conduct various audits, provide education to staff in nursing and other departments as needed
• Handling building issues and emergency code situations after hours and weekend.

Staff Nurse

• Provide evidence-based and client-centered care based on on-going assessment of residents’ conditions, evaluate outcome, and update care plans as necessary
• Collaborate with other health care professionals for clients’ best interest and well-being
• Provide education and therapeutic counseling to residents and families
