• Provides comprehensive case management services to all participants including intake and needs assessments, goal setting, progress monitoring, advocacy, and referrals with the goal of ensuring the identified students are successful in school.
• Supervises Case Management Tutors including scheduling and payroll management.
• Facilitates professional development for the Case Management tutors.
• Works with a cohort of identified students conducting individual check-ins to monitor well-being, attendance, and academic progress based on referral or performance in the classroom.
• Connects students and their families to available school and community resources.
• Develops and maintains resource connections with external and internal partners.
• Conducts outreach and service coordination activities.
• Completes administrative duties such as attendance tracking, filing student documents, and updating student check-in notes.
• Maintains appropriate student and family files.
• Collaborates with school administration in planning events and organizing parent workshops to support the students.
