- Manage and supervise a casual pool of 20-35 respite workers.
- Maintain a family and individual-centered intake process to determine the appropriate respite support required.
- Ensure families and staff understand and adhere to the service parameters outlined in the policy and procedures documents.
- Identify the needs of the client and family and coordinate services with a respite worker who can best support the team.
- Create referrals to other agencies when need identified.
- Coordinate and participate in formal and informal meetings with community professionals to share information concerning clients’ needs and progress.
- Update and maintain the respite introductory training modules to address best practices and relevant information for workers.
- Maintain client’s files with the most updated current health records.
- Provide regular support to staff and families, including suggestions to address support issues and status on hours.
- Compile and generate statistical and financial data for funders quarterly and at fiscal year-end.
- Liaise with the GNWT PFCG Pilot Coordinator to compile the program data and client support hours.
- Provide orientation to the caregivers on the PFCG Policies and Procedures Manual.
- Provide quarterly and fiscal year-end reports for the GNWT PFCG Pilot Coordinator.
- Provide and review the caregivers' care plans and allocated hours form.
- Assist with direct client caregiving support during staffing vacancies.
- on client request, contact agencies and professionals regarding issues related to the client.
- Follow up on behalf of clients to other agencies.
- Work directly with clients to determine their needs, taking clients to medical appointments, banks and community engagements.
- Provide information and research regarding clients’ needs.
- Provide support with documentation, forms and applications to various agencies.
