Provide continued care and supervision of program participant included but not limited to toileting, personal care, feeding, transfer and ambulation training. Assist program participant to/from transportation vehicle when arriving at or leaving the center. Maintain weights and blood pressures on assigned participant. Assist with the planning, set-up and implementation of individualized activity plan. Assist with the necessary maintenance of the program area.
